For the upcoming 2020 season, Portage County Emergency Medical Services will attend games when possible, and we will have on-site medical personnel.  While we hope to never need them, it will be great to have them there.  Thank you Portage County Emergency Medical Services!

Players are required to have completed one of the following to be able to participate in YAF:

1. Completed a physical in the past 2 years.  If a physical is needed, please reach out to Stevens Point Orthopedics at (715) 344-0701 or your local physician.
2. If the physical was completed in the last 2 years, an Alternate Year Physical Card is required to be filled out.  Please complete the form below and bring to the combine or your coach.
